Care Coordinator #921

The Care Coordinator is responsible for providing a wide range of clinical and administrative services to improve client care within the medical and psychiatric setting; assisting in the coordination and linking of clients to medical and psychiatric services; and working to optimize service delivery by assisting with and facilitating improved access to clinical care across agency programs as part of a strengths-based, consumer-centered approach.

Location: This position is based out of LifeWorks NW's Hillsboro Walnut site.

Pay/Benefits: $23.45 starting hourly pay, plus Full benefits.

Responsibilities

  • Provide Wellness Assessments to all new and existing clients of Lifeworks NW to support clients in engaging with providers to meet holistic health and wellness goals. Also includes identifying primary care providers (PCP) and oral health providers.
  • Use population segmentation and four quadrant model to identify clients in varying risk categories and provide targeted strategies to reduce risk and improve overall health and wellbeing.
  • Participate in ED reduction strategies by tracking ED visits and providing outreach post discharge.
  • Coordinate referrals with other providers in a timely and professional manner. Collaborate with interdisciplinary team and community services to ensure that any diseases, as well as preventative and health promotion needs, are addressed as part of the holistic, whole-person, culturally sensitive approach.
  • Assist identified individuals with accessing needed physical and oral health care by bringing their situation to the attention of their treatment team and attending clinician.
  • Work with interdisciplinary team to promote coordination of service delivery and prevent duplication of services ? including direct care coordination, skills training, and case management services.
  • Administer outcome measurement tools and participate in data collection, including basic health screenings (blood pressure, weight, height readings, BMI calculations, etc.).
  • Assist staff working with identified individuals with overcoming any barriers impeding access to physical and oral health care and supporting clients in maintaining health.
  • Track referrals and their status on identified metrics to determine project effectiveness.
  • Develop relationships with appropriate staff at medical and dental providers to assist treatment teams and attending practitioners as they help individuals access needed services and coordinate care.
  • Identify care systems and services to assist in coordination of care with appropriate partners such as primary care providers, oral health providers, or other medical providers in consultation with team and supervisors.
  • May provide cross coverage for other care coordinators on the team as well as psychiatric care coordinators.
  • Maintain confidentiality of records relating to clients' treatment.
  • Meet professional agency standards for documentation and accuracy of work, client care/customer service, attendance/punctuality, and teamwork as defined in the Performance Evaluation.
  • Document all required client information into agency-specific Electronic Health Records system and maintain confidentiality with Personal Health Information; participate in site/program/team specific tracking systems such as reports and spreadsheets.
  • Continually increase level of cultural sensitivity, awareness, and competency.
  • Participate in regular individual and team meetings with supervisor. Provides written and verbal reports to supervisor regarding cases. Attends informational and clinical meetings, which may include all-agency meetings, team meetings, consultation meetings with psychiatrist, staffings, intra-agency trainings, etc.
  • Travels between sites or in the community when needed.
  • Participate in a culture of feedback at the agency by eliciting feedback to promote professional growth and assess work effectiveness.
  • Use Feedback-Informed Treatment service delivery approach to obtain and review feedback from clients regarding the fit and effectiveness of the services provided to them.

Required Qualifications

  • A bachelor's degree in a behavioral science field from an accredited college/university OR three (3) years of full-time professional experience working with adults experiencing mental illness (preferably with adults experiencing severe and persistent mental illness.)
  • Demonstrate the competencies outlined in OAR 309-016-0005 to become credentialed as a Qualified Mental Health Associate (QMHA) at the time of hire.
  • Demonstrates cultural competency and trauma informed care in addressing barriers which may lead to lack of access and engagement.
  • Good computer skills including a functional knowledge of Microsoft Outlook, Word, OneDrive, Excel and Teams. Ability to quickly learn proprietary software including Electronic Health Record system.
  • Ability to perform essential functions of job without creating a direct threat to the safety of self or others.

Preferred Qualifications

  • The CADC credential is desired given the high occurrence of substance use/abuse.
  • Clinical experience providing outpatient services or working in community health, with primary care is preferred.
