Employment Case Manager, QMHA 1018

This Employment Case Manager, QMHA position will work at LifeWorks NW's multi-service site located in Beaverton, Oregon and will also assist clients with their employment needs in addition to providing case management. This may include providing job coaching, assessing clients for employment capabilities, assisting clients with job search, developing job leads in the community, and other duties as assigned.

LifeWorks NW's Assertive Community Treatment (ACT) team is a multidisciplinary team providing treatment to those with severe and persistent mental illness diagnoses and most at risk for civil commitment, incarceration and psychiatric hospitalization.

Purpose: Assists with the implementation of treatment plans, provision of individual and group skills training, case management, and coordination of multiple service providers for client caseload under the direction of a senior clinician. Provides services onsite, within the community, and within clients' homes. Keeps accurate and timely records of activities.

Pay/Benefits: $18.81 to $20.45 per hour. Full benefits.

Hours: This is a full-time position; also responsible for the ACT 24-hour crisis phone one week out of every 7 weeks.

Essential Responsibilities: 

  • Meets standards for documentation and work accuracy including timely completion of progress notes, assessments, treatment plans, authorization requests.
  • Meets standards for direct service, implementation and utilization of the clinical model, quality of clinical practice and teamwork.
  • Responsible for client orientation including advising clients and significant other(s) of the services offered through the program and recommend services.
  • Provides input on clinical assessments and the development of treatment plans.
  • Provides case-coordination by maintaining contact with other agencies, referral source(s) and staff regarding client and significant other(s) involvement in treatment services.
  • Conducts individual and group skills training sessions based on current evidenced based.
  • Teaches socially acceptable behavior, employing techniques such as behavior modification and positive reinforcement. Encourages individuals and family members to develop and use skills and strategies for confronting their problems in a constructive manner.
  • Observes and influences clients' behavior, communication and interaction. Detects unusual behavior and reports observations to professional staff.
  • Organizes, supervises, and encourages clients to participate in social, educational, and recreational activities. Provides assistance and support to clients with the job search process.
  • Assists the client to the extent necessary, based on client's level of functioning, to access needed services and resources. Advocates on client's behalf. May attend the client's psychiatric medical appointments, if clinically indicated.
  • Reviews and evaluates clients' progress in relation to measurable goals described in treatment plans.
  • Acts as a client advocate to coordinate required services or to resolve emergency problems in crisis situations. Provides clinical support to clients with immediate and urgent needs
  • Provides assistance with 'team tasks' (taking responsibility in one particular area i.e. arranging transportation).
  • Continually increases level of cultural sensitivity, awareness and competency.
  • Maintains chart notes related to external contracts, obtains pertinent records from other treatment providers, and tracks client's involvement, follow-through and progress in varied treatment activities. Completes documentation in electronic health record system in a timely manner.
  • Transports clients in a personal and a LifeWorks NW vehicle.

Qualifications:

  • Bachelor's degree in Psychology, Social Work or related behavioral health field or at least three full years of similar mental health work experience required. 
  • This position requires credentialing through Mental Health & Addiction Certification Board of Oregon (MHACBO). All prospective candidates must have a valid MHACBO registration number upon hire.
  • Requires access to a car, a valid driver's license, proof of current automobile insurance coverage, and ability to meet LifeWorks NW's driving requirements.
  • Demonstrated ability to provide case management services for people with severe and persistent mental illness in compliance with confidentially policies.
  • Demonstrated ability to communicate effectively; understand mental health assessment, treatment and service terminology and individual, group, family and other counseling techniques; program policies and procedures for services and supports identified in an Individual Services and Supports Plan.
  • Demonstrated ability to provide psycho-social skills training and implement elements of a treatment plan.
  • Knowledge of community resources and treatment agencies enabling appropriate referrals for specific client needs.
  • Proficient in Microsoft Outlook and Word. Ability to master proprietary software including electronic health record system and scheduling system required.
  • Knowledge or experience of employment coaching techniques, i.e. developing job leads, assisting clients with resumes and interviewing, and addressing employment barriers strongly preferred but not required.
