Fit Case Manager #394

LifeWorks NW's Project Network program is a residential program with a focus on the African American community for women in recovery dealing with alcohol/drug issues who are with their small children.

The FIT Case Manager serves as the primary liaison between the substance abuse treatment team and the FIT for Recovery collaborative team members:  Child Welfare, Juvenile Court staff, Outreach Workers, ART screeners, etc.  Provides case management services, offering a combination of referral linkage, individual and family support, group counseling and case management services to assist client's successful engagement in treatment, the family's ability to support the client's continued treatment involvement, and the client's long-term successful recovery from substance abuse. 

Location: This position is based in NE Portland. 

Pay/Benefits: $21.32 starting hourly pay, plus Full benefits

ESSENTIAL RESPONSIBILITIES:  

  • Works with the FIT for Recovery Collaborative to ensure a successful course of treatment and resolution of the Child Welfare case.
  • Screens clients and determines case management needs.
  • Develop a Service Plan that facilitates each client's successful completion of his/her Individual Services and Supports Plan (ISSP).
  • Facilitates engagement in services.  The FIT Case Manager is the primary source of communication between treatment services and Outreach Workers.
  • Maintains clear and collaborative communication with FIT for Recovery partners, including Child Welfare, The Juvenile Court, Attorneys, Treatment staff, Outreach Workers, Parent Mentors. Maintenance of these relationships includes completion and monitoring of status change forms, monthly status reports, and attendance and participation in meetings and court appearances.
  • Provides case management services at least monthly to each client (more often as needed), as indicated in the Service Plan, including locating and accessing needed resources, removing barriers to treatment success, facilitating movement between levels of care, vocational and educational coaching, assisting in the development of recovery-oriented support systems, and skills training.  Case management services are offered in multiple settings, including the community and at home. 
  • Complete documentation in electronic health record system.
  • Check e-mail/voice mail daily when working.
  • Attend informational and clinical meetings which may include all-agency meetings, team meetings, consultation meetings with psychiatrist, staffing, intra-agency trainings, etc. as required.
  • Some evenings/weekends required.
  • Travels between sites or in the community, including providing client transportation.

REQUIREMENTS: 

  • Substantial experience with issues involving addiction including the language, values, norms and other attributes of the subculture; the subtleties and linguistic nuances of the differences between honest dialogue and statements made for effect.
  • Extensive knowledge of how to efficiently navigate the social service network from the bottom up.
  • Must be able to accompany clients throughout their environment. 
  • Must have knowledge of the legal/corrections system.
  • The ability to maintain confidentiality and appropriate boundaries.
  • Demonstrates effective communication skills and maintains a cooperative working relationship with others in a culturally diverse environment.
  • Good computer skills including a functional knowledge of Outlook and Word.  Ability to master proprietary software including electronic health record system.
  • Requires ability to work some evenings and/or weekends
  • Requires access to car, valid driver's license, proof of current automobile insurance coverage and verification of safe driving record from DMV.
  • Ability to perform essential functions of job without creating a direct threat to the safety of self or others.
