Care Coordinator/QMHA 922

This position will work at a LifeWorks NW's multi-service site located in Beaverton, Oregon (Portland metro area).

The Care Coordinator position provides a wide range of clinical and administrative services to improve client care within the medical and psychiatric setting; to assist in the coordination and linking of clients to medical and psychiatric services; works to optimize service delivery by assisting with and facilitating improved access to clinical care across agency programs as part of a strengths-based, consumer-centered approach.

This position is currently providing telehealth services using Zoom technology through the COVID-19 pandemic. This position will be 100% office-based with some possibility of community-based services as medically necessary. 

Days/Hours: This is a full-time position working 40 hours a week. The times are generally 8:30 am to 5:00 pm. 

Pay/Benefits: $19.00 to $20.66 per hour. Full benefits

Essential Responsibilities:

  • Using information provided by a variety of sources, identify LifeWorks NW clients with chronic health conditions (eg diabetes and SPMI) that are inadequately engaged with physical and oral health providers.
  • Administers outcome measurement tools and participates in data collection, including basic health screenings (blood pressure, weight, height readings, BMI calculations, etc.)
  • Work with inter-disciplinary team to promote coordination of service delivery and prevent duplication of services.
  • Coordinate referrals with other providers in a timely and professional manner. Collaborate with inter-disciplinary team and community services to ensure that any diseases, as well as preventative and health promotion needs are addressed as part of the holisitic, whole-person, culturally sensitive approach.
  • Assist identified individuals with accessing needed physical and oral health care by bringing their situation to the attention of their treatment team and attending clinician.
  • Assist staff working with identified individuals with overcoming any barriers impeding access to physical and oral health care and supporting clients in maintaining health.
  • Track referrals and their status on identified metrics to determine project effectiveness.
  • Develop relationships with appropriate staff at medical and dental providers to assist treatment teams and attending practitioners help individuals access needed services and coordinate care.
  • Identify care systems and services to assist in coordination of care with appropriate partners such as primary care providers, oral health providers or other medical providers in consultation with team and supervisors.
  • Provide direct care coordination, skills training, and case management services.
  • Maintain confidentiality of records relating to clients' treatment.
  • Meets standards for documentation and accuracy of work as defined in the Performance Evaluation.
  • Meets standards as defined in the Performance Evaluation for client care, quality/customer service, attendance/punctuality and teamwork.
  • Continually increase level of cultural sensitivity, awareness and competency.
  • Participates in regular individual and team meetings with supervisor. Provides written and verbal reports to supervisor regarding cases. Attends informational and clinical meetings, which may include all-agency meetings, team meetings, consultation meetings with psychiatrist, staffings, intra-agency trainings, etc. 
  • Checks e-mail/voice mail daily/throughout the day when working responding to contacts within 48 hours for routine matter (urgent matters may require a more immediate response)
  • Travels between sites or in the community when needed.

Qualifications:

  • Bachelor's degree in a behavioral health field (Psychology, Counseling, Social Work, etc.) and/or three full years providing mental health related services.
  • This position requires a valid and active QMHA credential certification through MHACBO upon hire.
  • Experience with and knowledge of the physical health system, including a basic understanding of terminology, and a general understanding of Medicaid and Medicare.
  • Demonstrate effective communication skills and ability to provide culturally competent treatment services to diverse client populations.
  • Good computer skills including a functional knowledge of Microsoft Outlook and Word. Ability to master proprietary software including electronic health record system.
  • Requires access to a car, a valid driver's license, proof of current automobile insurance coverage and verification of safe driving record from DMV.  
  • Must be able to pass a DHS background check.
