LifeWorks NW provides intensive outpatient services for adults with substance use disorders. This position supports the men's intensive outpatient program.
The Recovery Mentor helps to engage (adult male-identified) clients in treatment, orient them to the program, support their early recovery needs, conduct needs assessments and assists clients with resources. Position also provides outreach who are at risk for substance use or are currently using. Offers a combination of referral linkage, family support and case management services to assist client's successful engagement in treatment and the family's ability to support the client's continued treatment involvement.
This Recovery Mentor position must be able to conduct observed UAs on adult male-identified clients. Because of this requirement for this specific position, only male candidates can be considered.
Days/Hours: This is a part-time position working 20 hours per week. Specific days/times are To Be Determined. Will involve evening work at least up until 7:00 PM on some days. Position is Monday through Friday only.
Pay/Benefits: $17.02 to $18.30 per hour. Pro-rated benefits
- Screens and selects clients to participate in the mentoring program.
- Develop the service plan for all clients including a service reporting plan on each referral.
- Within the first five working days following intake into services, the client and (if appropriate) family members meet with the Recovery Mentor and the Primary Counselor to develop a case management plan.
- Should have daily contact with clients for up to the first five weeks in the program helping with engagement in recovery support, housing and employment issues, family adjustment issues, legal, housing, medical and benefit issues. Frequency of contact should gradually decrease to the point of twice monthly approximately 90 days in the program.
- Responsible for assisting the family in the initial implementation of the plan, maintaining regular communications with the client's primary therapist, collateral providers, community corrections staff, and linking the client system to needed services within the community.
- Determines when to terminate clients from the program because services are no longer necessary, or the client is not responding to the mentor model.
- Meets regularly with staff from social service agencies to update them on LifeWorks NW services identifying unmet services/needs LifeWorks NW may be able to address; review and update referral and case coordination procedures as needed; obtain referral requests and work with referral sources to remove any barriers to treatment; report back if clients fails to attend; obtain relevant case information from the referral for the clinician who will be doing the client's intake assessment; work with referral source and client to facilitate the family's involvement in the assessment and the treatment planning process.
- Meets standards for documentation and work accuracy including timely completion of progress notes, assessments, treatment plans, and authorization requests.
- Meets standards for direct service, implementation and utilization of the clinical model, quality of clinical practice and teamwork.
- Transport clients in a personal vehicle to meetings/appointments within LifeWorks NW and within the community.
- This position must be able to conduct observed UAs on adult male-identified clients. Because of this requirement for this specific position, only male candidates can be considered.
- Must be a Certified Recovery Mentor or have completed a Recovery Mentor approved training in the state of Oregon.
- Must have a letter from the Office of Equity and Inclusion (OEI) with the Traditional Health Worker (THW) Number: https://www.oregon.gov/oha/OEI/Pages/THW-Become-Certified.aspx. If interviewed, must be able to provide a copy of the letter.
- Must be able to drive clients in a personal vehicle, have a valid driver's license, current auto insurance, and ability to meet LifeWorks NW driving requirements.
- Substantial experience with issues involving addiction including the language, values, norms and other attributes of the subculture; the subtleties and linguistic nuances of the differences between honest dialogue and statements made for effect.
- Extensive knowledge of how to efficiently navigate the social service network from the bottom up.
- Must be able to accompany clients throughout their environment.
- Must have knowledge of the legal/corrections system.
- The ability to maintain confidentiality and appropriate boundaries.
- Demonstrates effective communication skills and maintains a cooperative working relationship with others in a culturally diverse environment.
- Basic computer skills including keyboarding, use of mouse, ability to navigate website and use common web browser.
- Must be able to pass a DHS background check.
